Pulled over, underground desert mental hospital

My older brothers girlfriend (who isn’t his gf in real life, I actually didn’t recognize her face) was driving my brothers car in the middle of no where it was a New Mexico looking desert. She got pulled over by a cop and my brother and her were kinda pissed and got out of the car to talk to the cop. Me and my friend stayed in the car. After a while of talking a minivan slams on its brakes and the driver (a very old man) slams his head on the steering wheel. Me and my friend were like wtf. Then he gets out of his car waves at us and sprints into the desert over a hill leaving his car running with its door open. I try and convince the cops to go after him but they don’t care. I get out and go after and my friend, although hesitant follows me. After a little bit of searching my brothers girlfriend shows up saying she couldn’t leave us alone out here. Then I see him and yell for him to come back to his car. He starts running and I chase him, he leads me to a really deep hole with a ladder going down, I couldn’t see the bottom. He goes down the ladder so I follow him. Right as I get down there this guy with a light on his head (bioshock infinite looking dudes) attacks me with a wrench but I counter it and start running. Everywhere I go there are crazy people trying to harm me, most aren’t wearing shirts and some have lights on their heads others look like orcs.
